P16 and Ki67 Immunostains Decrease Intra- and Interobserver Variability in the Diagnosis and Grading of Anal Intraepithelial Neoplasia (AIN)

Abstract: Background: Significant variation is reported in the diagnosis of HPV-associated AIN. We previously observed that bandlike positivity for p16 in >90% of contiguous cells coupled with Ki67 positivity in >50% of lesional cells is strongly associated with high grade AIN. This study was undertaken to determine if addition of p16 and Ki67 immunostaining would reduce inter- and intraobserver variability in diagnosis and grading of AIN.Design: H&E stained slides of 60 anal biopsies were reviewed by three pathologists and consensus diagnoses were achieved: 25 negative, 12 low (condyloma and/or AIN I) and 23 high (9 AIN II and 14 AIN III) grade lesions. The H&E stained slides were diagnosed independently by three additional (“participant”) pathologists. Several weeks later they re-examined these slides in conjunction with corresponding p16 and Ki67 immunostains.Results: Addition of p16 and Ki67 immunostains reduced intra- and interobserver variability, improved concurrence with consensus diagnoses and reduced two-step differences in diagnosis. Negative and high grade AIN diagnoses showed the most improvement in concurrence levels.Conclusion: Addition of p16 and Ki67 immunostains is helpful in the diagnosis and grading of AIN.

Engineering education in the wake of hurricane Katrina

Abstract: Hurricane Katrina struck the gulf coast on August 29, 2005. This natural disaster catastrophically affected the U.S. gulf states and negatively impacted the entire country. My place of residence, Baton Rouge, Louisiana, survived hurricane Katrina with serious but non-catastrophic damage because the edge of the 40 mile wide eye of the storm passed 90 miles to the east of the city.I experienced many events analytically and from an engineering perspective, for example, the doubling of our city population, traffic gridlock, loss of communication (two days), loss of power (three days), and a gas shortage (none for five days, shortage for five weeks). Volunteering in the aftermath of Katrina provided many more experiences that were primarily emotional. I volunteered at the University of New Orleans satellite office that was established on the Louisiana State University (LSU) campus just after the hurricane. Employees were supposed to call in with contact information and their evacuation location; about half the calls I fielded were from relatives of the employees who were trying to ensure that their loved ones were okay. On the rare occasions that a loved one's relative had already contacted us, I could inform the caller that the person was safe, but I wasn't allowed to provide the person's phone number for liability reasons.School classrooms and boy and girl scout troops from all over the country sent school supplies for displaced children; while unpacking these supplies, our public school distribution team read heartfelt words of hope and kindness that frequently brought us to tears.The LSU AgCenter livestock barn, the Parker Coliseum, was turned into a holding area for pets evacuated from the storm. Abstract: this chapter identifies "context minimization error" as the tendency to ignore the impact of enduring neighbourhood and community contexts on human behaviour. the error has adverse consequences for understanding psychological processes and efforts at social change. the chapter describes a series of theoretical models of how neighbourhoods and community settings are associated with various aspects of human welfare and reviews evidence of associations of context with health, psychological distress, risk behaviours, psychological attitudes, and child development. it suggests that many psychological processes may play out differently in different contexts and that contextual factors interact with socio-cultural characteristics of individuals in predicting outcomes. people, in turn, can shape community contexts. a more sophisticated understanding of the effects of contexts depends on more sophisticated approaches to assessing them.

Abstract: The author explains how the revolution in the way new technology can reproduce, disseminate, and store digital information, including copyrighted works, is truly a double-edged sword for authors and right-holders. The challenge of copyright in the digital age is to preserve the author’s and right-holder’s incentive to create new works and use new technologies to distribute them to users and consumers in the face of a huge competitive threat from the illicit use of technology by infringers. Finally, the author warns about the debate that new technologies often prompt about whether the set of exclusive rights granted to authors and right-holders should be modified, either with new or broadened rights or new or broadened exemptions, to continue to serve the purpose of copyright.

Abstract: Mirror-image symmetry in limbs is normal in the vertebrate phenotype. Genetic and epigenetic factors regulate the differentiation, patterning, and development of the embryo and foetus. Growth after birth is influenced by hormonal and environmental factors such as nutrition. Limb size asymmetry in a child should trigger a search for associated pathology that may include neoplastic conditions, sequelae of injury, vascular, and neurogenic factors. Macrodactyly, part of the PIK3CA Related Overgrowth Spectrum, offers the physician a clinical challenge, while at the same time an opportunity to study morphology, histology, and more recently the molecular mechanisms from which the conditions arise. Collaboration between clinicians and basic scientists offers an exceptional opportunity for coordinated study and the potential for improved patient outcomes

Abstract: The older adult population is rapidly growing expecting to reach nearly 95 million by 2060. Consequently, there is a growing need for young adults preparing to enter the workforce to have accurate perceptions of the aging process. The purpose of this study was to examine students’ verbatim descriptorsof someone over 65 years of age and how they change after completingan undergraduate health and aging course. 132 junior-level Health Behavior Science undergraduate students completed a pre-course and post-course questionnaire that assessed aging knowledge and perceptions. A thematic analysis was conducted to examine students’ descriptors of individuals 65+ pre- and post-course. An evaluation of the qualitative responses indicated that the undergraduate students’ perceptions of older adults and aging changed from their pre-course to post course responses. Positive personality trait descriptors increased from 160 (11.5%) initially to 268 (31.8%) following course completion. Conversely, negative traits of being sedentary/inactive (pre (4.6%) to post (0.5%)), being slow (pre (4.8%) to post (2.5%)), having health problems (pre (9.7%) to post (4.5%)), and being fragile (pre (7.5%) to post (5.3%)) all decreased. Independent samples paired t test showed a significant increase (p<0.0001) in aging knowledge scores from pre to post course. In year one, the scores increased from 64.45 (+12.63) to 78.66 (+12.67) and in year two the scores increased from 69.41(+13.12) to 80.75 (+11.07). Overall, this study demonstrated that an undergraduate health and aging course with a service-learning component greatly impacted undergraduate students’ perceptions of older adults and the aging process to be more positive.
